To streamline your visit, please bring a valid photo ID and any necessary documentation related to your services. If your visit involves a workplace-related injury, we ask that you also bring any relevant paperwork from your employer, including authorization for treatment.
If you’re taking any medications, please come prepared with a list or documentation of these, as this information helps our healthcare team provide you with the most appropriate care. For those with requirements such as corrective vision, remember to have any necessary aids, like glasses or contact lenses, with you during exams.
In certain cases, additional testing may be necessary to fulfill compliance requirements. Our team will guide you through this process if it applies to you.

Obtain Authorization from Your Employer
What is Authorization? For all employer-sponsored services at CADUCEUS, a formal authorization is required. This should preferably come as a Service Request via our proprietary EMR system. We also accept Email and Fax (for urgent needs and high volume occasions only), or Paper (for after-hours visits only).
*Reminder: We do not accept verbal authorizations. If your employer has questions about this process, please direct them to our Service Request portal for a smooth experience.
Receive and Confirm Your Authorization
Depending on the authorization type, your employer might have set an appointment or advised you to come in as a walk-in.
If you receive the authorization by email or fax, please be prepared to present it when you arrive.
Scheduling Your Appointment
If you’re responsible for scheduling, contact your preferred clinic directly with your authorization in hand.
If your employer has used a Service Request, they may have set an appointment and, if your email address was provided, you’ll receive a confirmation email.
TPAs may also coordinate directly with our clinic to confirm appointments for employees.
On Appointment Day
Bring a valid ID (Driver’s License, State ID, or Military ID) and any necessary payment form, as CADUCEUS is a cashless facility.
National Network Appointments
Authorization:
CADUCEUS will coordinate with your employer for services outside Georgia or Missouri.
Typically, your employer will submit a Service Request, which our team processes promptly.
Self-Pay Options:
For certain services, such as sleep studies, we offer Self-Pay arrangements via credit card. Once payment is confirmed, we schedule the service.
TELEMEDICINE Appointments
Authorization:
If your employer provides access to our telemedicine services, they authorize virtual visits directly through our TeleMed app or platform.
Alternatively, if your employer prefers CADUCEUS to arrange this, we will request an email confirmation and apply a nominal administrative fee for setup.
Self-Pay Options:
Private Virtual Visit (Self-Pay): Our telemedicine app offers a self-pay option (coming soon!) where patients can independently book and pay for virtual visits.
After Your Appointment
For follow-up, test results, or future appointments, our team will contact you via SMS, email, or through your employer. If any further action is required, please consult with our staff during your visit.
As a patient who has paid for services rendered, you have the right to receive copies of your medical records. To ensure timely delivery, please provide a valid email address at the time of your visit. CADUCEUS will automatically send your visit summary directly to you.
For employer-sponsored Corporate services, the medical record is the property of the employer who authorized your visit.

If you’ve received injury care at a CADUCEUS facility or through telemedicine, you may request access to your medical records at any time. Your first request is complimentary; subsequent requests may incur a fee. If your employer uses a carrier, you may receive an SMS requesting your claim number.
Accessing my Medical Records
We’ve done our part to make requesting your medical information easy. With your request—through CADUCEUS EMR, email, or pick up —we'll provide you with the provider's notes, medical test results, and lab reports.
To use this system, you must successfully enter the patient's first name, last name, social security number, date of birth, and date of injury. The fields must match an electronic medical record or patient visit in our system. system. You will be given three (3) opportunities to submit this information correctly, after which your session will be locked.
Records not found in the electronic request may be requested via email at medicalrecords@caduceusoccmed.com.

We strongly encourage you to use our request form to expedite the processing of the records delivery (immediate delivery). If we have a manual request via mail or fax, please allow 3 – 5 business days for processing the request. In certain instances, it can take up to 30 days.
