Pre-Visit Forms
To ensure a smooth and efficient visit, all required forms should be completed in advance and presented at the time of service.
An authorization is required for CADUCEUS to provide services.
• Preferred Method (Recommended): Submit an authorization through the 'Service Request' feature in the CADUCEUS EMR.
• Alternative Method: Provide a completed Paper Authorization Form, filled out by the employer, to the clinic at the time of service.
If the patient is under 18, a parent or guardian must complete the 'Authorization for Minors' Form before services can be provided.
To help us deliver the best care, patients must complete the Patient Information Sheet (Face Sheet).
This form captures essential personal and medical details and should be presented at the time of service.
