If the patient is under 18, a parent or guardian must complete the ↳Authorization for Minors Form before services can be provided.
To help us deliver the best care, patients must complete the ↳Patient Information Sheet (Face Sheet).
This form captures essential personal and medical details and should be presented at the time of service.
